Advance Care Planning Workshop

HOUGHTON — UP Health System Home Care and Hospice’s Julie Beck, LBSW will be holding a series of 4 work sessions on Advance Care Planning for your health. These sessions will be held at the Portage Lake District Library on Sept. 14, 30, and Oct. 12, 28, 2021 from 3 to 4 p.m.

At the first workshop on Sept. 14, Advanced Care Planning education and information will be presented. Forms that are legal in the state of Michigan will be available. The goal for Session 1 is to see that you have the forms and information you need to start the process. We will talk about identifying the person you would like to have make health care decisions for you if you could not.

Session 2 begins with the knowledge that you have identified the person who will be making health care decisions for you, when you can not. This workshop would be an opportunity to bring the person you choose with you. The session will be very informal and is meant to support you as you begin to look at the choices you have and have conversation about what those choices might mean for you. The presenter will work with you to review the form and ask questions to help you clarify your thoughts.

Sessions 3 and 4 will remain opportunities to continue the work you have already completed and possibly even finalize your document. The presenter will support you through to completion of the form, share with you the next steps and who you will want to provide copies to.
